Well we received a call from our home study agent. She has sent us a packet of forms to fill out. Most need to be notarized, so here are the following forms so far that we are in the process of completing:
- A D.S.S. form
A SLED background check and clearences
- Personal background info for the both of us
- 3 names and address and phone #s with 3 reference Letters ( non- family)
- Complete medical forms filled out
- List of all previous address
- 6-8 pictures of our house and family
- Financial statement
- Copys of Birth records, marriage certificate, and tax returns
So we are getting started and are setting up a date to meet with the social work to start the home study. On top off all of the paper work, am going to get my lip checked out! Its has been going numb for a month now, kinda scared. Dont really want to know whats causing it, but David is making me go :(
